In partnership with the Colorado Health Foundation, OACS is now offering Advance Care Planning education and hands-on assistance in the creation of Advance Directives at no charge! Advance Care Planning (ACP) is the process of making decisions about the healthcare that might be needed if you were unable to speak for yourself. These decisions are based on personal values, preferences, and discussions with loved ones.
Only 25% of Americans have put their medical care wishes in writing.
- They believe that Advance Directives forms can only be completed by an attorney
- They believe they are not at “that” age and, therefore, do not need to worry about it yet
- They just don’t want to think about illness and death
The truth is that more than 50% of people are actually unable to participate in end of life decisions when the time comes because they are too sick, suffering from dementia, or unconscious. Families, ministers, and loved ones are left trying to decide how the person would want to be cared for at the end of their life. Planning ahead is the key to peace of mind.
Through this initiative, LFS hopes to equip older adults, their families, church leaders, and local professionals with the tools necessary to make decisions about future medical care to better direct in times of trial.
